The DPSST PS-1 form is required for all new applicants to obtain the basic certificate to perform work as a security professional or private investigator in the State of Oregon. Click the form to open it for printing. Please read the instructions carefully and do not sign page 7 until you are in front of the notary public. Included in this form is DPSST Form PS-27 form, the Private Security Provider Code of Ethics.
The DPSST PS-7 form is the Training Evaluation form for evaluating your instructor. By having attendees complete this form it will help us improve our professional instructors and make adjustments when necessary.
The DPSST PS-21 form is required anytime certification renewals are submitted for any certification including Basic Security Provider and Refresher, Private Investigator, Instructor, Supervisor or Manager.
The DPSST PS-27 form is the Private Security Code of Ethics including core values, good character, fair treatment of others, and respect for the laws of the State of Oregon and the nation. This is now part of the PS-1 form.
The DPSST Allegations of Violation Form is for filing an allegation of a violation or violations to DPSST.
The DPSST PS-4 form is required and will be provided by your instructor. The form is an affidavit to be completed by the person who rolls your fingerprints. It must be completed, signed and included in the tamperproof package with your fingerprints and your PS-6 form, which is also provided by the instructor.
The DPSST PS-20 form is your temporary work permit and must be completed by your supervisor or manager. Your copy of the form must be carried on your person at all times when you are on duty until you receive your DPSST card. In addition, one copy remains with you employer and one copy sent to DPSST.
The DPSST PS-23 form to inform DPSST of your change in employment, name, address, phone number, or email address.
The DPSST Criminal Charges Form under ORS 181.885 anytime you are charged with a crime, you are required to inform your employer or if not currently employed, you are required to notify DPSST within 48 hours.
The DPSST Credit Card Authorization Form allows DPSST to charge your credit card for charges you authorize.

The DPSST PS-6 form is not available online and will be provided by your instructor once you successfully complete the course.
